Here is information for both participants & parents:
The Parent Permission Slip is the paperwork each participant must fill out. These papers were given out at the Mini-THON kickoff on January 29th. An additional online registration must be completed at http://fourdiamonds.donordrive.com/index.cfm?fuseaction=donorDrive.event&eventID=739.
**A $10 registration fee AND a minimum of $25 must be raised by a student in order to participate in the event.**
The T-Shirt Order Form was also distributed at the Mini-THON kickoff.
